How We Interface
with Our Clients' Hospital Networks
CareNex
Care Coordinators and Case Managers function independently with
their own high-tech data collection and communication devices.
They work collaboratively as they interface with hospital-based
personnel (case managers, nurses, discharge planners, social service
representatives and others) in Neonatal Intensive Care
Units and Special Care Nurseries of hospitals on behalf of our
clients.
They are
highly-experienced in working with both hard copy and electronic
medical records as they review and abstract a variety of source
data. These include medical records, physician and nurse
notes, respiratory care notes, discharge planning, social service
and OT/PT notes.
Communication and
coordination of services between hospital-based case management and
payor-based case management/utilization review is emphasized
throughout the workflow process.
The highly-detailed chart
review by CareNex Care Coordinators and Case Managers enhances
our clients' ability to assess ongoing medical necessity and
authorize ongoing inpatient days in a timely
fashion.

and redundant paperwork for the hospital staff.
CareNex Care Coordinators and Case Managers can often assist
